Evaluation and Assessment Services offered:

• Neuropsychological Evaluations for purposes of determining specific neurological impairment secondary to traumatic brain injury or head trauma, ischemic attack, dementia and memory impairment, toxicity, seizure disorder and other causes (recommendations will be determinant of source and reason for referral);
• Neuropsychological Evaluations for purposes of worker's compensation, independent medical examinations, forensics purposes, and pain management (i.e. SCS, ITP, DRG);
• Psychological Evaluations for purposes of determining specific cognitive impairment, impairment in daily functioning, problems with focus and concentration, developmental delays, etc. Recommendations will be determinant of source and reason for referral;
• Psychological Evaluations for purposes of bariatric surgeries; and
• Psychological Evaluations for purposes of professional assistance, i.e. TLAP and TNPAP.
  

Noncovered services by insurance, arrangements of payment to be made prior to service:

• Psychological Evaluations for purposes of professional assistance, i.e. TLAP and TNPAP; and
• Neuropsychological Evaluations for purposes of worker's compensation, independent medical examinations, or forensics evaluations requested for legal purposes (to be referred by attorney).
